29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a. On or about January 28, 2015, at Building D, front entrance to the building, an employee being transported up and down from the roof was not wearing his body belt and lanyard attached to the basket.  He was being exposed to fall hazards up to 25 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about January 28, 2015, Building D, front balcony, the employee working on the balcony installing metal plates, was not protected from falling 25 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method.  The Home Star Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 988603, citation number 01, item number 001 and was affirmed as a final order on September 9, 2014 , with respect to a workplace located at 62 N Shingle Lane, Water Sound, FL 32413.
