OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee were exposed to the hazard of amputation.  a) BG-3 Catwalk adjacent:  On or about February 2, 2015  The employer did not ensure employees did not keep two hands on the bandsaw when reaching to cut overhead PVC piping.  Among other methods, feasible and acceptable means of abatement is to:    Follow the manufacturers Operator's Manual such as but not limited to:  Personal Safety Section: Do Not overreach.  Keep proper footing and balance at all times.  Specific Safety Rules Section: DANGER: Keep hands away from cutting area and the blade.  Keep second hand firmly on the handle.
