29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  a)  Welding Shop:  On or about February 3, 2015, and at times prior thereto, employees were exposed to the hazards of combustible and respirable dust, from the accumulation of aluminum oxide dust on the floor area surrounding the sand blaster, while sand blasting.  b)  Assembly Shop:  On or about February 3, 2015, and at times prior thereto, employees were exposed to the hazards of combustible dust, from the accumulation of sawdust on the area surrounding the Delta radial arm saw, while assembling parts.

29 CFR 1910.212(a)(3)(ii):  The point of operation of machines whose operation exposes an employee to injury, was not guarded:   a) Machine Shop:  On or about February 3, 2015, and at times prior thereto, employees were exposed to an amputation hazard from the lack of a guard on the unused portion of the Rob, Inc. vertical saw blade, while they sawed off pieces of wood and steel.  b) Machine Shop:  On or about February 3, 2015, and at times prior thereto, employees were exposed to an amputation hazard from the lack of a guard on the Enco drill press, while they drilled holes into pieces of sheet metal.  c) Assembly shop:  On or about February, 3, 2015, and at times prior thereto, employees were exposed to an amputation hazard from the lack of a guard on the Delta radial arm saw, while they cut pieces of lumber.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  Sheet Metal Shop:  On or about February 3, 2015, and at times prior thereto, employees were exposed to a struck by flying projectiles hazard such as a shattering abrasive wheel, where the work rest was greater than 1/8-inch from the abrasive wheel of a Baldor 8-inch bench grinder, while touch-up grinding drill bits.

On or about February 3, 2015, and at times prior thereto, employees were exposed to a struck by flying projectiles hazard, and also the potential of air entering the bloodstream, from operating the compressed air hose  at approximately 111 p.s.i., while cleaning off dust and wood debris from the radial arm saw operation.
