29 CFR 1910.180(b)(2): Crawler, locomotive, and truck cranes constructed and utilized on or after August 31, 1971, did not meet the design specifications of the American National Standard Safety Code for Crawler, Locomotive, and Truck Cranes, ANSI B30.5-1968, which was incorporated by reference as specified in Sec. 1910.6. Crawler, locomotive, and truck cranes constructed prior to August 31, 1971, should be modified to conform to those design specifications by February 15, 1972, unless it can be shown that the crane cannot feasibly or economically be altered and that the crane substantially complies with the requirements of this section.    (a)  Yard:  On or about February 3, 2015 and times prior thereto, Astoria Marine Construction was using two cranes that were manufactured in 1905 and 1915.  These cranes did not meet the design specifications of ANSI B30.5-1968.  This exposed employees to struck-by hazards from the crane having structural issues while lifting a load.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    Big Building:  On or about February 3, 2015 and times prior thereto, the control panel for the compressor operating at 120 volts was disconnected and the wires were accessible to employees.  This exposed employees to severe shock/burns if employee had incidental contact with the wires.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  (a)  Big Building:  On or about February 3, 2015 and times prior thereto, the electrical disconnect for the planer was missing a knock-out.  This exposed employees to potential electrical shock/burn hazard from incidental contact with live electrical parts.  (b)  Big Building wood shop:  On or about February 3, 2015 and times prior thereto, there were two electrical junction boxes by the drill press were missing there covers.  This exposed employees to potential electrical shock/burn hazard from incidental contact with live electrical parts.  (c)   Big Building glue room:  On or about February 3, 2015 and times prior thereto, there was electrical junction box that was uncovered exposing live electrical parts This exposed employees to potential electrical shock/burn hazard from incidental contact with live electrical parts.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)  Fabrication shop:  On or about February 3, 2015 and times prior thereto, the Grizzly horizontal band saw connection did not have proper strain relief to protect it from pulling on the wires.  This exposed employees to potential shock hazard from the wires grounding out.   (b)  Fabrication shop:  On or about February 3, 2015 and times prior thereto, the flexible cord  connection did not have proper strain relief to protect it from pulling on the wires.  This exposed employees to potential shock hazard from the wires grounding out.

29 CFR 1915.502(a): A written fire safety plan was not been developed and implemented by the employer that covered all of the actions necessary to ensure the employees safety in the event of a fire.  (a)  Shipyard:  On or about February 3, 2015 and times prior thereto, the employer did not have a written fire safety plan in place.  This exposed employees burn/smoke inhalation hazards from not being trained in requirments for fighting fires.
