29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE:   a) On February 2, 2015, employees working on top of a water tank were not protected from falling to the lower level, exposing employees to a fall hazard of approximately 12 feet.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     a) On February 2, 2015, an employee was hospitalized at the Guam Memorial Hospital as a result of falling 12 feet from a work platform and the employer did not report the incident until February 6, 2015.
