29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent:  (a) Casino Barge Walkway Structure - On or about January 21, 2015 tags were not posted to warn employees not to walk near railing on each side of walkway due to cracks caused by water damaged and decay.  Supervisor verbally warned employees to stay 4 to 5 away from the edges after accident.

29 CFR 1915.158(b)(1): - When work was being performed on floating vessels 200 feet or more in length, at least three 30-inch U.S. Coast Guard approved ring life buoys with lines attached were not located in readily visible and accessible places:  (a) Casino Barge - On or about January 27, 2015 employees worked on barge 300' long that did not have 3 life rings.  (b) Crane Barge - On or about January 27, 2015 employees worked on barge 200' long that did not have 3 life rings.
