29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not be protected by protective helmets:   a)  On February 19, 2015, in front of the house, head protection was not worn while working below employees carrying and using hammers and trowels to smooth out the concrete design features on the roof, exposing employees to the hazard of being struck by the tools.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:   a)  On February 19, 2015, in front of the house, there was no midrail or equivalent in the guardrail system for about 12 feet along the roof edge. Employees brought sand up to the roof at the location without the midrail and worked about ten feet from the location without the midrail to smooth out the concrete design features on the roof.
