29 CFR 1910.146(d)(4): The employer did not provide the equipment specified in paragraphs (d)(4)(i)-(d)(4)(ix) of this section at no cost to employees, did not maintain that equipment properly, and/or did not ensure that employees use that equipment properly.    a) Thomas Concrete of Georgia Plant # 500, Wash Grade Sand Silo # 2; 4650 Canton Road, Marietta, GA: The employer did not provide and/or ensure employees used equipment properly in that employees inside the silo were utilizing a single rope as a lifeline, which they used to secure themselves (both employees used the same rope at the same time).  The employees were engulfed with sand and one employee became disconnected from the rope, which resulted in a fatality.
