29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;    a) On or about Feb 16, 2015 the company did not select and have press operators to use appropriate personal protective equipment in that the press operators did wear foot protection with metatarsals attached for the movers of metal coils.

29 CFR 1910.136(a): General requirements. The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards.    a) On or about February 16, 2015 foot protection was not provided and/or worn during the installation of metal coils exposing employees to foot injury from fall metal coils.    b) Safety shoes were not provided and/or worn for the employees that move the metal coils in the facility including but not limited material handlers.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  a) On or about February 16, 2015 while preparing to position a metal coil for installation onto the mandrill, a machine operator was not protected from struck-by hazards in that the coil was not secured to prevent movement/ tipover and coil moved falling upon the employees foot.
