29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.       At the facility; the employer failed to report an employee work related injury that resulted in hospitalization within twenty-four hours to OSHA.

29 CFR 1910.27(f): All ladders were not maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure.      At the receiving area; employees accessed an unsafe fixed 11 feet ladder,  exposing employees to fall hazards.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.      At the store front bathroom; an employee was not adequately trained on the physical and health  hazards associated with hazardous chemicals such as but not limited to Triple Play which contained didecyl dimethyl ammonium chloride, and Multi Surface and Glass Cleaner with Peroxide, exposing employees to inhalation hazards.
