29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:      2117 Blossom Lane, Arnold: An employee was exposed to a fall distance of approximately 16 feet from a ladder jack platform where the employee did not use required personal fall arrest systems or guardrails systems while working.

29 CFR 1926.452(k)(2): Ladders used to support the ladder-jack scaffolding did not meet the requirements of Sub-part X:      2117 Blossom Lane, Arnold:  An employee was exposed to fall hazards where the employer used a non-ladder on the right side of a ladder jack scaffold.  The effective fall distance to level was measured at approximately 16 feet.
