29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:      a)  On or about March 2, 2015, employees were not protected against accidental contact with energized overhead power lines during framing and carpentry work along east elevation of the new addition to the house.  The overhead power lines consisted of secondary lines having a voltage rating of 120/240 volts.       No abatement certification or documentation is required for this item.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).   a)  On or about March 2, 2015, employer permitted to erect and use pump jack scaffolding approximately two feet away from the energized power lines having a voltage rating of 120/240 volts.    No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a)  Employees were not protected from the hazard of falling approximately 7.5 feet to the lower level while performing carpentry work around unguarded floor opening.     No abatement certification or documentation is required for this item.
