29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:   Along the production line floor, on or about March 5, 2015, and at times prior thereto, employees were exposed to being caught by from unsecured rotating projected shaft ends measuring approximately 5" in length by 2" in diameter of a CMS chemical coater applicator roller machines model CM-634-HS, while a continuous feeding of the sheet metal is being processed along the continuous production line.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a)  Along the south side area of the production floor, on or about March 05, 2015, and at times prior thereto, employees were exposed to electrical shock hazards of 240 volts from an open circuit breaker that was not completely closed.  b)  Along the north end of the production line, On or about March 05, 2015, and at times prior thereto, employees were exposed to electrical shock hazards of 240 volts from open electrical knockouts measuring approximately 1" while working around equipment such as but not limited to, lighting systems, mixing tanks and recoiler machines.
