29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about March 10, 2015, at 204 Creekside Crossing, New Braunfels TX, the employer did not ensure employees were protected from falling.  The employees exposed to a fall hazard in two areas while trying to install windows.     a) Employee exposed to a six foot fall from the upper ledge to the lower roof.  b) Employee exposed to a twelve foot fall to the ground below from the lower roof.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   On or about March 10, 2015, at 204 Creekside Crossing, New Braunfels TX, the employer did not ensure employees were using the six foot a-framed self supporting step ladder properly. The employees were permitted to walk on the top step to gain access to the building. Employees exposed to a fall hazard.
