Worksite located at 3001 S. Providence Road, Columbia, Missouri- Employees were observed working and walking directly beneath elevated materials, equipment and other employees and were not protected from impact by the use or hard hats.

Worksite located at 3001 S. Providence Road, Columbia, Missouri- Employees were exposed to fall hazards in that employees were observed working on top a an awning and were not provided any means of fall protection.

Worksite located at 3001 S. Providence Road, Columbia, Missouri- Employees were exposed to fall hazards in that ladders being used to access the roof and awning were not extended at least 3 feet above the upper landing surfaces.
