Employees on this site were exposed to hazards including, but not limited to fall and struck by hazards.  Condition noted on or about 03/12/15. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a- 110 Ashley Avenue, Lakewood NJ: Employees, not supplied with fall protection, were exposed to fall hazards ranging from approximately 9 feet 7 inches to approximately 23 feet 3 inches to the ground surface below when performing exterior sheathing installation and other related activities from window openings and various roofs with varying pitches of/on a new residential structure.  These employees had not received fall protection training from the company.  Condition observed on or about 03/12/15.     NOTE: IN ADDITION TO ABATEMENT DOCUMENTATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.  FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     a- 110 Ashley Avenue, Lakewood NJ: An employee was exposed to eye and face injuries when they were not wearing any eye/face protection when cutting 4 x 8 sheets of composite board sheathing with an electric circular saw.    b - 110 Ashley Avenue, Lakewood NJ: An employee was exposed to eye and face injuries when they were not wearing any eye/face protection when using a pneumatic nailer to secure sheets of composite board sheathing to the exterior surface of a residential structure.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    a- 110 Ashley Avenue, Lakewood NJ: Employees, not supplied with fall protection, were exposed to fall hazards of approximately 23 feet 3 inches to the ground surface below when performing exterior sheathing installation and other related activities from a residential structure's main roof with a pitch of approximately 10 on 12.  Condition observed on or about 03/12/15.     b- 110 Ashley Avenue, Lakewood NJ: An employee, not supplied with fall protection, was exposed to fall hazards, of approximately 14 feet to the ground surface below, when performing exterior sheathing installation and other related activities from the roof of a pitched awning (approximately 6 on 12) on the new residential structure.  Condition observed on or about 03/12/15.     NOTE: IN ADDITION TO ABATEMENT DOCUMENTATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.  FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

Condition observed on or about 03/12/15.     NOTE: IN ADDITION TO ABATEMENT DOCUMENTATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.
