29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a- 12 Division Avenue, Surf City NJ: An employee, not utilizing eye/face protection, was exposed to struck by hazards when operating an electric circular saw at the worksite.  Condition observed on or about 04/16/16.   Exclusive Carpentry Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1076552, citation number 1, item number 1 and was affirmed as a final order on 12/30/15, with respect to a workplace located at 248-20 Cambria Avenue, Little Neck NY 11362.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 12 Division Avenue, Surf City NJ: An employee, not utilizing fall protection, was exposed to fall hazards of approximately 9 feet to the ground surface below when performing roof sheathing installation, and other related activities from a first story roof with a pitch of approximately 2 on 12.  Condition observed on or about 04/16/16.   b- 12 Division Avenue, Surf City NJ: An employee, not utilizing fall protection, was exposed to fall hazards in excess of 18 feet to the ground surface below when walking/working at unprotected edges of a residential structure's second story deck.  Condition observed on or about 04/16/16.   Exclusive Carpentry Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1076552, citation number 1, item number 2a and was affirmed as a final order on 12/30/15, with respect to a workplace located at 248-20 Cambria Avenue, Little Neck NY 11362.
