29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Laminator # 1: An employee was exposed to an amputation hazard from unguarded rollers of the Laminator # 1 machine. On 3/12/2015, an employee was injured by the unguarded rollers of the Laminator # 1 machine. Lamart Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 944153, citation number 1, item number 1 and was affirmed as a final order on 1/21/2014, with respect to a workplace located at 16 Richmond Street, Clifton, New Jersey 07015.
