29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    On or about March 16, 2015, and prior thereto, CCA Building, the cover on the exhaust fan mounted on the west wall had openings of greater than one-half inch.  This condition exposed employees to contact with the rotating fan blades.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)  On or about March 16, 2015, and prior thereto, employees using skids extending from between poles to climb to the top of the stack of poles on buggies were exposed to the hazard of falls from elevation.  Employees were not using personal protective equipment that included a full body harness, lanyard and anchor point.    (b)  On or about March 16, 2015, and prior thereto, employees standing and walking on top of poles stacked on buggies to place and remove skids and place and remove chains were exposed to the hazard of falls from elevation.  Employees were not using personal protective equipment that included a full body harness, lanyard and anchor point.    Or in the alternative,    OSH Act of 1970 Section 5(a)(1)    (a)  On or about March 16, 2015, and prior thereto, employees using skids extending from between poles to climb to the top of the stack of poles on buggies were exposed to the hazard of falls from elevation. Failure to do so exposed employees performing servicing and maintenance work to the hazard of being caught by or between moving parts and contact with the moving saw blade.    Treated Materials, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 944828, citation number 1, item number 6a and was affirmed as a final order on April 3, 2014, with respect to a workplace located at 6040 Highway 79 North in Rison, Arkansas.
