29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Arena Hub Plaza: Employees were not protected from falling approximately twenty-two (22) feet to the ground level while performing roofing activities from a flat roof approximately 120 ft wide by 150 feet long, observed on or about 3/24/2015    Biscontini Roofing, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 917027 citation number 1, item number 1 and was affirmed as a final order on September 27, 2013, with respect to a workplace located at 601 North Laurel Street, Hazleton, PA 18201

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Arena Hub Plaza: Employees engaged in roofing operations were not trained to recognize fall hazards and the procedures to be followed in order to minimize these hazards while working from a roof approximately twenty-two (22) ft above the ground level, on or about 3/24/2015.
