29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about March 24, 2015, employees on the ground assisting the lifting of standing seam sheet metal roof sections were not provided with head protection, exposing employees to the hazard of being struck by falling pieces of sheet metal.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about March 24, 2015, four employees installing the sealed beam sheet metal roof on the commercial property under construction were not protected from falls overs 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about March 24, 2015, employees working on the commercial property under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of approximately 16 feet.
