29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors: On the east side exterior of the project, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection when employees used a 110 VAC Makita circular saw, powered through an extension cord, to cut lumber, exposing employees to the hazard of contacting electricity.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws: On the east side exterior of the project, the outside insulation was not connected to the receptacle on an extension cord that supplied power to a 110 VAC circular saw used to cut lumber, exposing employees to the hazard of contacting electricity.
