Such machine, tool, material, or equipment was not either identified as unsafe by tagging or locking the controls to render it inoperable or physically removed from its place of operation:  At the project next to the accounting office, a 10 inch table saw, Skilsaw, model number 3400 13A 966H, with no hood guard, was not either identified as unsafe by tagging or locking the controls to render it inoperable or physically removed from its place of operation. Employees that used the saw to cut 0.5 inch thick plywood for shelves were exposed to the hazard of striking against the blade.

Employees that used the saw to cut 0.5 inch thick plywood for shelves were exposed to the hazard of striking against the blade.
