Dai Ichi Apartments Roof Project: An employee was at the edge of the three-story high roof assisting and watching a coworker engaged in water blasting the roof surface in preparation for painting.  The employee was not protected by a personal fall arrest system, nor other methods to prevent a fall to the ground below.

The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  Dai Ichi Apartments Roof Project: An employee  engaged in water blasting the roof surface in preparation for painting was equipped with and using a harness as part of a personal fall arrest system which was not properly sized.  The harness was too large and the attachment point of the body harness was below the center of the wearer's back.  The employee was exposed to internal and external injury in the event of an extended period of suspension in the system due to its improper size.
