29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about 4/3/15, employees in a trench with an approximate depth of 6'5" did not have a safe means of egress to leave the trench when backfilling and bedding drain tile.  The ramp on the south end of the trench was not smooth and continuous and the trench itself was approximately 100 feet in length, allowing employees to work further than 25 feet from any form of egress.
