Main St. Cortland, Ohio 44410 the employer did not ensure Maxill, Inc employees were protected from electrical hazards by creating and implementing a hazardous energy control program. Employees routinely are exposed to electrical shock while replacing receptacles and light fixtures.

On or about April 7, 2015 at the work site located at 500 W. Main St. Cortland, Ohio 44410 the employer did not ensure portable containers into which hazardous chemicals were transferred were labeled nor intended for immediate use. Multiple containers were observed without labels. The containers of chemicals were for the cleaning of different types of surfaces by the cleaning crew whom had not been on site for more than a day.
