29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:      The front of the residential home under construction, east: On or about April 09, 2015, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet while installing clay roofing tiles on a residential home.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     The front of the residential home under construction, east: On or about April 09, 2015, and at times prior thereto, employees were exposed to a fall hazard while using a ladder, that did not have the side rails at least 3 feet past the roof landing, to access and install clay roofing tiles.
