The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: When employees were required to perform servicing and maintenance the employer did not establish a program for the control of hazardous energy (lockout/tagout) for training and periodic inspection for those related undocumented procedures. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: The employer did not have adequate point of operation guarding to prevent employees from having any part of their body in the danger zone during the operating cycle on the following machines: a) On Press #13- Safety switch on the horizontal injection molding machine was bypassed. The safety switch was zip tied to the closed position. This exposed employees to struck by and burning injuries from the plastic injection molding process. b) Guillotine Shears- Two hand controls were mounted behind the cutting action of the shear. 2 hand controls need to be placed at a safe distance from the point of operation. This exposed employees to an amputation and struck by injury. c) On Press #15 - Guarding devices on the horizontal injection molding machine were not properly applied to prevent exposure to the pinch point created near the top edge of the guard when the platens close. This exposed employees to struck by and burning injuries from the plastic injection molding process. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: The employer failed to install and use equipment in accordance with the instructions listed and permitted uses: a) Cincinnati Milcron- The electrical power panel was left open. This exposed employees to a fire and/or shock hazard. b) Electrical power panel near Press #7- The employer failed to cover unused openings on the electrical enclosure. Employees were exposed to inadvertent contact with energized parts and fire hazards. c) Microsonic Plastic Grinder #14- The Start/Stop button was damaged and recessed into the electrical panel. This exposed employees a shock and fire hazard due to accidental contact with wires. d) Microsonic Plastic Grinder #11- The Reset button was damaged and recessed into the electrical panel. This exposed employees to a shock and fire hazard due to accidental contact with wires. e) 240volt outlet near Cincinnati Milcron- The employer failed to cover unused openings on the electrical enclosure. Employees were exposed to fire and inadvertent contact with energized parts. f) EZ Surf II Bridgeport- Electrical panels were left open. This exposed employees to fire and/or shock hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
