29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:  (a)  At 16-A, Curington Farms, Enterprise, AL: On APRIL 10, 2015 and at times prior to; the employer exposed employees to electrical shock hazards of 220 VAC from a service pole that had an open faced circuit breaker panel.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On Lot 16-A, Curington Farms:  On April 10, 2015 and at times prior to; the employer exposed employees to falls between 10.5 and 11.8 feet while affixing asphalt shingles on a 7:12 pitch roof, in that employees were not protected from falls.
