29 CFR 1926.300(b)(1): When power operated tools were designed to accommodate guards, they were not equipped with such guards when in use.   a) At the project on or about April 10, 2015, outside the parking lot area, there was no guard on a Makita right angle grinder used to cut a line on asphalt road, exposing employees to the hazards of being struck by the wheel and small rocks.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.   a) At the project on or about April 10, 2015, outside the parking lot area, a black flexible cord supplying electrical power to a Makita Jack Hammer was not provided with strain relief.  This caused outer insulation and inner wires to be directly transmitted to the plug joints or terminal screws, exposing workers to the hazards of contacting live electricity.
