29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) News Vehicle Garage- The employer has failed to maintain all places of employment in an orderly condition.  The walking surface on the News Vehicle Garage side of the North Overhead Door had a large crack and was not flat and free of tripping hazards.    b) News Vehicle Garage- The employer has failed to maintain all places of employment in an orderly condition.  The walking surface in the building side of the North Overhead Door located in the News Vehicle Garage had a "pothole" and was not flat and free of tripping hazards.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):     a) News Vehicle Garage- The employer did not provide a standard railing that included a midrail and toprail, or the equivalent, on all working platforms to prevent employee(s) from falling into adjacent dangerous equipment or ground level, in areas such as the top of all News Vans, when performing servicing and maintenance operations.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    a) News Vehicle Garage- The employer failed to ensure that flexible cords, such as cords for the News Vans in parking stall 14East, were supplied with strain relief to prevent pull from being transferred to joints.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
