29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1).  Warehouse:  On or about April 8, 2015 and times prior thereto, the Forest Supervisor did not ensure that the Dewalt angle grinder have the guard in place.  This exposed employees to potential struck-by hazard if the wheel were to break during use.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly.   BICC Building:  The Forest Supervisor did not ensure that Fire extinguishers in the warehouse were inspected monthly.  This exposed employees to smoke inhalation hazards from potential delays from having a fire extinguisher not working.
