The bench grinder was located in the fire cache area of the Burns Interagency Communication Center.  The Forest Service was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 978154, notice number 2, item number 2 and was affirmed as a final order on 12/15/2014, with respect to a workplace located at the Bend Pine Administration Site.  The Forest Service was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 312393465, notice number 2, item number 6 and was affirmed as a final order on 8/18/2011, with respect to a workplace located at the Lookout Mountain Ranger District.
