Site - Employees were not protected from falls of approximately 22 feet when the side rails of an aluminum extension ladder used to gain access to and from the roof was not positioned at least 3 feet above the roofs eave.

Site - Employees operating a pneumatic nail gun while installing shingles were not provided with eye protection.      AMC Quality Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 893295, citation number 001, item number 001 and was affirmed as a final order on 09/16/2014, with respect to a workplace located at Country Club Dr. Manchester, NH 03101.
