29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a. Site- An employee using a personal fall arrest system while working from a roof was not protected from striking the ground, 9 feet 7 inches below the eave. The employee was observed working at the eave with a 6 foot shock absorbing lanyard. The combination of stretch from the lanyard, slack in the lifeline, and the employee connected to the d ring at their back would allow for the employee to strike the ground.  AMC Quality Builders, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 893295, citation number 1, item number 2b and was affirmed as a final order on May 2, 2013, with respect to a workplace located at Country Club Drive, Manchester, NH 03101.
