29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.  a- 535 Mountain Ave., New Providence, NJ 07974:  Employees surveying work operations while walking across the concrete foundation were exposed to holes that were not covered, exposing employees to tripping hazards.     Condition was observed on or about 04/10/2015.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, were not guarded to eliminate the hazard of impalement.  a- 535 Mountain Ave., New Providence, NJ 07974:        1- Employees stepping up onto the open concrete foundation surveying work operations were exposed to the unguarded ends of reinforced steel protruding up from the ground circumferencing the outside edge of the foundation.       2- Employees entering and exiting the west end of building # 1 surveying work operations were exposed to the unguarded ends of the reinforced steel ends protruding up from the ground.    Conditions observed on or about 04/09/2015.
