29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) BIA, Division of Transportation, Building 40, Fort Defiance, AZ: On or about April 15, 2015 and at times prior thereto, a window-mounted air conditioner was plugged into an extension cord to supply power to the unit. The air conditioner power cord had a warning label stating "Do not use an extension cord or an adapter plug".
