29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  a) BIA Maintenance Yard, Building 128 (Parts Warehouse), Fort Defiance, AZ: On or about April 15, 2015 and at times prior thereto, monthly visual inspections were not performed. The last monthly inspection was 5/2012. There was only one portable fire extinguisher in the parts warehouse and its gauge indicated it needed to be recharged.

The bulbs were 76 inches above the floor and in close proximity to storage on shelves.
