Workers involved in exterior siding work were exposed to falls of approximately 33 feet to the ground below as they were working from the aerial lift's basket's top guardrail without using any form of fall restraint or protection. Worker was exposed to 22 foot falls from the third floor balcony to the ground below as the portable ladder being used to access the balcony did not have its side rails extend at least 3 feet above the balcony surface.
