29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition whenever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:  a)  Jobsite -On or about April 23, 2015 an employee was working with a powered screw gun without protective eye wear while another employee without protective eyewear was handing cut metal roofing up to another employee in an aerial lift above him to hand to the employee with the screw gun.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Jobsite- On or about April 23, 2015 an employee was working beneath an aerial lift handing cut metal roofing to another employee in the aerial lift without wearing protective gear for his head.

29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, shall be provided for each 3,000 square feet of the protected building area, or major fraction thereof. Travel distance from any point of the protected area to the nearest fire extinguisher shall not exceed 100 feet:  a)  Jobsite - On or about April 23, 2015 an employee was working on the roof of the building and another employee in an aerial lift without having a fire extinguisher available to this contractor anywhere on the jobsite.
