Location: 1 Boatwright Loop, Plymouth MA:   A portable step ladder was being used on unstable sandy soil, without being secured to prevent accidental displacement as required, resulting in a fall.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  LOCATION: 1 Boatwright Loop, Plymouth MA:  The employee who fell from a step ladder, which he had set up on an unstable, sandy surface had not been trained by a competent person, in that training consisted of the employee receiving a booklet of rules written in a language he cannot read.
