29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. This violation occurred on or about April 03, 2015 when the employer did not train employees to recognize and avoid being struck by a T300 Bobcat being operated in the area. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure each employee is instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.

This violation occurred on or about April 03, 2015, at the site where an employee was injured and hospitalized as a result of a work-related incident that was not reported to OSHA within 24 hours. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that in-patient hospitalizations, amputations, or the loss of an eye caused by a work-related incident are reported within 24 hours.
