Shipping Gallery:  On or about April 23, 2015 and times prior thereto, an employee accessed a hatch to gain access to a lower level and was not wearing a fall protection harness.  This exposed employee to over a 50 foot fall to the lower level.  Shipping Gallery:  On or about April 23, 2015 and times prior thereto, an employee did not know how to properly wear his fall protection equipment.  He had the lanyard attached improperly and was not certain at what to tie off to.  This exposed employee to a potential fall hazard over 50 feet to the lower level.

(b)  Shipping Gallery:  On or about April 23, 2015 and times prior thereto:  A hatchway was opened and did not have proper guarding to protect employees.  This exposed employees to a 10 foot fall to the level below.
