29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 8-Jan inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:  6th Floor, Old House Bin Top Transition Area:  On or about April 23, 2015 and at times prior thereto, fugitive combustible dust greater than one-eighth (1/8) inch in depth had accumulated in the area.  This condition exposed employee to fire and smoke-related injuries related to the deflagration of combustible dust.

29 CFR 1918.22(a): A gangway at least 20 inches (0.51 m) wide, of adequate strength, maintained in safe repair and safely secured, was not used:  Dock area, Gangway:  On or about April 23, 2015 and at times prior thereto, the wire rope rigging used to move and support the gangway was damaged at lift points due to lack of wire-rope thimbles.  This condition distorted the wire rope land exposed employees to struck-by hazards from wire rope failure.
