Floor openings used for drainage when product spills and process tanks are washed down and drained were not adequately covered to prevent employees from accidentally tripping and falling.  Drain covers (metal grates) provided were damaged and/or missing sections leaving openings where employees could accidentally walk into them and/or trip over the damaged grates.  Observed on/or about 04/21/2015.
