29 CFR 1910.132(e): The employer did not remove from service defective or damaged personal protective equipment:    a) DSG Missouri, DBA Ashley Furniture @ 9791 Green Park Industrial Drive, St. Louis, MO 63123: On and before April 24, 2015, employees utilized order pickers to store and retrieve various furniture and home good items from vertical shelving units inside the warehouse. Employees utilized a damaged Rebel Protecta (model number AD  111A) self-retracting lifeline when they operated Order Picker serial number 1A385567. Approximately 2 feet of the inner polyester webbing was frayed. This condition exposed employees to a fall hazard up to and in excess of 30 feet.
