29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 04/30, 2015, at the above addressed worksite, employees installing blocks at the roof level of a building were exposed to a fall hazard of approximately 20-feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about 04/30/2015, at the above addressed worksite, employees installing concrete blocks at the edge of a building were using an extension ladder that did not extend at least 3-feet above the edge of the roof exposing them to a fall hazard of approximately 20-feet.
