29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees.  a) 602 Seventh Street - Worksite- On or about April 22, 2015, employees were exposed to electrical shock from the use of an extension cord that had numerous cuts in the outer jacket sheathing, exposing the inner conductors.  b) 602 Seventh Street - Worksite - On or about April 22, 2015, employees were exposed to electrical shock from the use of an extension cord which lacked strain relief placing tension on the terminals of the plug assembly.  c) 602 Seventh Street - Worksite - On or about April 22, 2015, employees were exposed to electrical shock from the use of an unmounted metal junction box with damaged knock out.   d) 602 Seventh Street - Worksite - On or about April 22, 2015, employees were exposed to electrical shock from the use of an extension cord which was bound with electrical tape due to having a damaged plug.
