29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:     The employer is failing to ensure that bundles consisting of dies and jigs are being carried by forklifts in the facility are stabilized and/or centered to prevent the load from shifting and falling.  The loads being carried by the forklift were rigged so as the load was "under slung" from the tines of the forklift.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:     The employer failed to report an amputation to OSHA within 24 hours of the incident occurring.  An employee of the company was moving a product with a forklift when the product fell striking the employee and amputating the tip of the finger.
