29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:     a) Sycamore Estates, Lot 12, Worksite -  on or about May 8, 2015, employees were working from a personnel platform lifted to a working height of approximately 18 feet above grade, which was not firmly secured to the forklift's lifting carriage and/or forks.
